If they’re referring to the DWC1 (click the link below to access the form) then the answer is simply “yes.” That is the official start of the Workers Compensation process. That form asks basic questions such as your name, where the injury occurred, your body parts that were involved, your social security number and things like that. And that’s then given to the insurance company which ultimately then starts the investigation process and the claims process. So the short answer is “yes, that DWC1 form needs to be filled out.”
